Workshop Registration Information

To register, complete the registration from and mail it, along with a check made payable to:

You may also register by fax (651-292-6065) or E-mail (drc-administration@attbi.com) and pay prior to the start of the workshop. Please complete one form for each person registering. Photocopy additional forms as needed. Payment vouchers or completed purchase orders will also be accepted. Workshop space is limited and attendance is determined on a first-come first-served basis. Please register at least one week prior to the workshop.
